Fill them in on your phone
Answer the health history questions and sign the consent forms with your finger. Your answers go straight to your chart, so there is nothing to print.
What the forms ask
- Your contact details and the reason for your visit
- Your health history, medications and past injuries
- A checklist of symptoms, so the doctor starts from the full picture
- Any consent form we need you to sign
- A place to add a recent MRI or blood-work report, if you have one
If something goes wrong
- The link keeps working until you have finished, so you can leave and come back to it.
- Did not get your link? Check your spam folder, or call your office and we will send it again.
- Would rather use paper? Ask for a paper copy at the front desk when you arrive.
